NL50 SR pot, flop XR and follow through

Villain is a nityy reg type player 13/8 over 215 hands. As such very limited info on his flop, turn and river tendencies.
Preflop ($0.75) Hero is BB with T Q
2 folds, CO raises to $1.50, 2 folds, Hero calls $1.00
Flop ($3.25) 5 J 8
Hero checks, CO bets $2.06, Hero raises to $6.00, CO calls $3.94
Turn ($15.25) 5 J 8 8
Hero bets $12.00, CO calls $12.00
River ($39.25) 5 J 8 8 2

Trying to go more in depth with my analysis of postflop situations so please be patient if my thought process is way off :)

My raising range on the flop is made up of made hands , including all sets (JJ, 88, 55) as well as the 2 remaining J8s (flop =11 combos, turn and river = 8 combos). To balance I have some strong combo draws QT, Q9, T9, 97 and 76 of hearts (5). For bluffs I use GSSD with possible BDF equity so QhTx (3) QxTh (3) Qh9x (3) QTcc Q9cc 97cc 64cc (4).

So value to bluff ratio on the flop of 11:13 with 5 semi bluffs. On the turn I want to continue to bet the 5 heart combo draws, my value range obv then for bluffs I think the QT combos and 64cc seem like my best options? This gives me 8 bluff combos.

Question is should I be jamming this combo on the river.

Plusses for this specific combo I block 3 QQ combos, 3 JT and 3 QJ combos from his calling range. Negatives are I do block Th FD combos T8, T9 and AT maybe, the others likely fold the turn already.

64cc seems like the most obvious river bluff choice then after that similar arguments can be made for the rest. Any suggestions or recommendations on how to approach these type of analysis spots welcome!
